Please read carefully!  This is a legal document that affects your legal rights!

 

For the purposes of this agreement, the client refers to the individual who receives salon services.

 

Due to the outbreak of COVID-19, The NAIL SPA is doing everything we can to protect our clients, our community and our staff.  We are following CDC guidelines with regard to social distancing practices and sanitation.  We use EPA-approved products to clean and disinfect.  We adhere to the Board of Barbering and Cosmetology’s and LA County Department of Public Health’s rules.  We ask that all clients follow CDC’s social distancing practices and truthfully disclose their health histories, with regards to the symptoms of COVID-19.  By doing so, the health and safety of the clients and the staff of The NAIL SPA can be ensured.

 

TERMS OF SERVICE:  The client confirms the following statements are true:

• Within 14 days of any visits, the client has not experienced any of COVID-19 symptoms, such as fever, cough, shortness of breath, sore throat, new loss of taste or smell, chills, head or muscle aches, nausea, diarrhea, or vomiting.

• Within 14 days of any visits, the client has not been in close proximity to anyone who was experiencing any of the COVID-19 symptoms or has experienced any of the COVID-19 symptoms since their contact.

• Within 14 days of any visits, the client has not been in close proximity to anyone who has tested positive for COVID-19. 

• At the time of any visits, the client has not been tested for COVID-19 and/or waiting to receive test results

• The client confirms that the client has not been tested positive positive for COVID-19, or are you presumptively positive for COVID-19.

 

ASSUMPTION OF RISK: The client recognizes that EVERYONE must comply with all set procedures to reduce the spread of the virus while attending the appointment.  The client acknowledges the contagious nature of COVID-19 and that The NAIL SPA cannot guarantee that the client will not become infected with COVID-19.  The client accepts the risks during the services at The NAIL SPA. 

 

RELEASE AND WAIVER:  The client agrees not to visit The NAIL SPA for any of the services if Terms of Service are not met.  The client acknowledges that all the information given is truthful, accurate and complete.  The client, hereby, releases and agrees to hold The NAIL SPA harmless from, and waive on behalf of him/herself, his/her heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to him/herself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from The NAIL SPA. The client understands that this release discharges The NAIL SPA from any liability or claims that he/she, his/her heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from The NAIL SPA. By signing below, the client confirms that he/she understands and agrees to all terms and statements in this form. This release and waiver of liability extends to the salon together with all owners, partners, and employees for any and all visits.
