All clients and employees who enter the spa are required to be screened for COVID-19. If the answers to any of the following questions are yes, The NAIL SPA will be glad to reschedule the appointment to after the 15th day of this date. 

 

Preventing the spread of COVID-19 is EVERYONE’s responsibility.

Are you currently experiencing, or have you experienced in the past 14 days, any of the following COVID-19 symptoms? Fever, cough, shortness of breath, sore throat, new loss of taste or smell, chills, head or muscle aches, nausea, diarrhea and vomiting.

In the past 14 days, have you been in close proximity to anyone who was experiencing any of the COVID-19 symptoms or has experienced any of the COVID-19 symptoms since your contact?

In the past 14 days, have you been in close proximity to anyone who has tested positive for COVID-19? 

In the past 14 days, have you been tested for COVID-19 and waiting for lab test result? 

Have you tested positive for COVID-19, or are you presumptively positive for COVID-19?
